ACCME Accreditation Statement

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Cardiovascular Institute of the South (CIS) and Saint Luke’s Foundation, Inc. CIS is accredited by the ACCME to provide continuing medical education for physicians.

AMA Credit Designation Statement

Cardiovascular Institute of the South designates this live activity for a maximum of 19.25 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.


The currency used is US dollar.

Banking hours are 0830-1600hrs, Monday to Friday.  Automatic Teller Machines are well spread throughout Maui.  International credit cards are accepted at most hotels and city stores.  The closet bank to the resort is ‘First Hawaiian Bank’ 10 minutes by taxi.

Carparking facilities at the Grand Wailea hotel are available via Valet.  Valet parking for Day use only is $10.00 to $60.00 per car, per day; or overnight parking is $30.00 per car, per night.

Please respect the presenter and other members of the audience by ensuring your mobile phone is switched off or to silent while you are in sessions.


It will be summer in Maui and the average temperature is 84 degrees F (29 degrees C).

Delegate List

The delegate list will be held at the Registration & Information Desk.  A delegate list will be distributed electronically to all registered attendees and exhibitors after the completion of the Symposium. The delegate list will contain the name, organisation and country of origin of registered delegates and exhibitors.

If you do not wish your details to be provided to sponsors or other delegates, please tick the appropriate box on the registration form.

The dress code for all Symposium sessions and social functions is smart casual.


The exhibition area in the Haleakala 4 & Gardens 4 Rooms and will be open for the duration of the Symposium.


It is strongly recommended that you purchase comprehensive travel insurance to cover the duration of your visit to Hawaii.


The United States Government does not refund sales tax to foreign visitors. Sales tax charged in the U.S. is paid to individual states, not the Federal government - the same way that VAT is paid in many countries.

The tax is usually not included in the total price.

The Maui Memorial Medical Center is located in Central Maui, approximately 45 minutes from the hotel. There is also a doctor on call in Whaler's Village, only a ten minute walk from the Resort.

Speakers’ Preparation

Speakers are able to upload their presentations in advance of their session at the AV desk and ask that you do this no later than 3 hours before your scheduled talk.

Special Needs

In compliance with the legal requirements of the ADA and the rules and regulations thereof, every effort will be made to accommodate persons with disabilities.  Attendees with special needs (i.e., special diet, handicap access, hearing impaired) should indicate this when registering for the conference, and notify the travel agent or hotel when making a reservation so that such arrangements can be made on their behalf.
